Forest took the lead on two separate occasions during the contest, threatening to claim a third consecutive Premier League victory over their hosts. However, Víctor Muñoz denied them all three points with a powerful equaliser in the 82nd minute, sparing Liverpool from what would have been a damaging home defeat.
The turning point of the afternoon came amid confusion at the substitutes' board. Iraola had been attempting to make a triple change but was unable to do so because the fourth official's electronic board had broken down and could not display the incoming players' numbers. While the delay dragged on, Liam Delap took a quick throw-in that led directly to a penalty, which Morgan Gibbs-White converted calmly to put Forest back in front.
Speaking afterwards, an exasperated Iraola made clear his frustration with the situation, insisting his planned substitutions could have prevented the incident altogether.
